Childcare Provider Information

In the event that my child is injured or ill, I understand that the Eel River Beach Club will attempt to contact the child’s parent(s)/legal guardian(s) or an emergency contact at the telephone numbers provided. I hereby delegate authority and grant consent in advance of any specific diagnosis or treatment to any of the agents, servants and/or employees of the Eel River Beach Club and any doctor, hospital or clinic to exercise their best judgement as to necessary medical/surgical treatment for my child(ren).
